Why does my makeup do this?

Most of the time when I put on concealer or foundation it makes my face look patchy. How can I never have this happen again?

All I do for my routine is moisturizer (The Porefessional Smooth Sip), let it dry, then use a brush to apply concealer (Clinque Beyond Perfecting Super Concealer), and a makeup sponge applicator to apply liquid foundation (Fenty Beauty Eaze Drop). I used to use a powder foundation instead, but it caused my skin to look even patchier.

I don’t know very much about skin care or makeup as I’ve never had an interest until lately, so any advice regardless of how obvious it may seem would be very helpful.
